A numbered document annex is a schedule inside the equipment order. The schedule gives every drawing, datasheet, test result and handover sheet a number, an owner and one channel. Two factories, one main contractor and a hospital committee cannot share a memory. First, the annex replaces memory with a register. Second, the register replaces trust with a status letter. According to ISO 13485:2016, documents are reviewed and approved before issue. In practice, three of four sign-off lines sit with parties who never enter the room, so 75% of the trail has to be readable without a site visit. The 2026 register is five carriers and five clause lines.

What Is a Numbered Document Set in a Hospital Equipment Order?

A numbered document set is a register that ties each sheet to a project code, a room, a unit serial and a revision letter. The register answers one question per row: is this the sheet the order was signed against? Four parties read the same row. Each reader needs 25% of the register written for their job. According to 21 CFR 820.181, a device master record must include or point to device specifications, production specifications, acceptance criteria, packaging data and servicing procedures. The production hold points in the order add the room. One master record per model covers every unit built to that model. A room that changes late touches a different file. For example, a moved gas outlet changes the interface sheet and the unit record, not the model file. The 2026 register therefore has two parents: the model file and the room file.

Why Do Forwarded Emails Stop Being a Record?

A forwarded email is a copy with no revision state and no auditable recipient list. A thread that passes through four mailboxes leaves four near-identical copies. Three of the four copies are not the issued sheet, so 75% of what a reader opens can be the wrong paper. According to ISO 13485:2016 clause 4.2.4, changes to documents are reviewed and approved by the function that approved the original. A forward carries no such approval. A comment becomes a sentence in a message, not a line in a file. In practice, a committee that asks what changed in week nine reconstructs the answer from memory. Each row states the revision, the date and the approving role. The scope of supply in writing and the register are signed on the same day, so the row and the obligation share a date.

Which Carriers Need a Number Before Signature?

A carrier is any document that changes a decision when the revision changes. Five carriers cover the decisions in a theatre order: unit datasheet, factory acceptance test record, room interface file, unit record and closeout index. Each carrier gets a number before the order is signed. A number added later competes with the email that already exists. According to EU MDR 2017/745 Annex II, the technical documentation of a device includes the device description, design and manufacturing information, and verification results. A factory holds that file already. The order needs the room half. Three of the five carriers are created after the equipment ships, so 60% of the trail is generated on site. A register that covers factory documents only covers two fifths of the project. A committee that buys on paper alone buys the smaller two fifths.

The Five Carriers in One Table

The table lists the five carriers, the number each one carries and the party that issues it.

Carrier Number pattern Issued by Required by
Unit datasheet PRJ-DS-ROOM-Rnn Equipment factory Before purchase order
Factory acceptance record PRJ-FAT-SERIAL-nn Factory with contractor witness At witnessed test
Room interface file PRJ-RIF-ROOM-Rnn Main contractor Before first anchor
Unit record PRJ-UR-SERIAL Hospital with supplier engineer At handover
Closeout index PRJ-CL-ROOM-nn Main contractor At completion

How Is a Datasheet Revision Round Numbered?

A revision round is a fixed window in which comments are collected, answered and closed with a status letter. Round one starts when the factory issues a numbered datasheet. Comments close in ten working days. The status letter is A for approved, B for approved with comments, or C for revise and resubmit. According to ISO 13485:2016 clause 4.2.4, the function that approved the original document also approves each change. A status letter is the approval. Silence gets no letter, and no letter means no approval. If the annex allows five rounds of ten working days each, the rounds consume 42% of a 120-working-day programme. Buyers who force the datasheet measurement conditions onto page one get shorter rounds. The 2026 annex writes that budget on page one, so a late comment carries a visible cost instead of a hidden one.

What Makes a Factory Acceptance Test Record Traceable?

A witness record is a one-page statement that a named person saw a named test on a named serial. The page carries seven fields: serial number, test identifier, instrument identifier, calibration due date, limit, result and witness signature. A record without the instrument field cannot be repeated. A record without the serial field cannot be attached to a unit. According to 21 CFR 820.180(b), quality records are kept for at least two years from release for commercial distribution. A theatre project needs a longer clock, so the 2026 annex asks for five years. One witnessed unit per family covers 25% of a four-unit delivery. Four witnessed units cover 100%. Most committees choose the first option, then discover the gap at the second unit. A remote factory acceptance test on camera needs the serial visible in frame, or the record proves a model was tested, not a unit.

How Does the Room Interface File Join Two Factories?

A room interface file is the drawing set that states where each factory’s equipment meets the building fabric. The file carries ceiling grid, anchor positions, service drop points, clearances, floor loading and the name of the party that owns each interface. One drawing set serves three trades: electrical, medical gas and structure. A revision that misses one trade leaves 33% of the readers working to an obsolete sheet. According to IEC 60601-1 clause 7.2.2, the equipment carries the model reference and the serial number on its own plate. The interface file can therefore quote a unit identity rather than a product family. A hole placed for a family drawing may miss the delivered unit by the width of a mounting bracket. The 2026 file is issued before the first anchor. The installation interface pack at handover closes the loop between the drawing and the plate.

Who Signs the Unit Record, and in What Order?

A unit record is the acceptance file for one serial, signed in a fixed order so the warranty has one start date. The order is receiving inspection, fixing removal, first power-on clearance, functional tests, training and handover. Four signatures close the file. Two of the four are collected on site, so 50% of the approvals happen inside the theatre. According to the MHRA guidance Managing medical devices, acceptance checks and the associated records belong to the hospital before first clinical use. A signature out of sequence breaks the warranty argument. A handover signature placed before power-on clearance lets the factory argue the unit was accepted untested. The 2026 record prints the six steps with the four signature boxes in the order above. A record with a blank step is not a record. A record with a dated blank step is a question.

Where Does the Chain Split Between Contractor and Committee?

A responsibility split is a written boundary that states which party issues a carrier and which party accepts the carrier. The split is a line, not a sentiment. According to ISO 14971:2019 clause 4.4, the risk management plan names who is responsible for each activity and the criteria used to judge acceptability. An equipment order can copy the same logic for documents. Three of the five carriers sit with the contractor, so 60% of the register is contractor-led. Two sit with the committee, which is 40%. A split written this way survives a change of staff. A split held in a meeting does not. In practice, the boundary is easiest to test with one question per row: who signs, and who may reject? A row with two possible signers has no owner. The 2026 annex answers both questions for every carrier.

What Does the Main Contractor Own?

The contractor owns the three carriers that depend on the building: the factory acceptance witness line, the room interface file and the closeout index. Ownership means the contractor issues the number, opens the revision round and answers comments inside ten working days. According to ISO 9001:2015 clause 8.5.3, property belonging to a customer is identified, verified, protected and safeguarded. A hospital room under construction is customer property. A drawing that moves a service drop without telling the hospital damages property. The contractor also owns the schedule that shows when each number is due. A register without dates is a list. The 2026 version puts a due date beside every row. Three of five carriers, or 60%, then have one clear owner. The unit acceptance record before payment stops drifting between parties, and the contractor knows which three dates the committee will measure.

What Does the Hospital Committee Own?

Committee ownership is the two carriers that release money and start the warranty: the unit record and the datasheet approval that precedes the purchase order. The committee signs as the accepting party. According to ISO 13485:2016 clause 4.2.5, records must stay legible, identifiable and retrievable for the lifetime of the device, and for not less than two years from release. A committee that signs and then files the record in a shared inbox fails the retrievable test. Two of five carriers, or 40%, carry a rejection right. The 2026 annex states the rejection window as ten working days. Silence after the window counts as rejection, not approval, so the register never assumes consent from a missing reply. The committee also owns the archive, because a record that leaves the building on handover day is never archived.

How Do the Numbers Reach Closeout and Warranty?

A closeout index is the final revision of the register, and the index becomes the payment condition. The index lists every carrier number with the last approved revision letter. A warranty clock that starts at delivery can run for months before the first case. If commissioning takes eight weeks of a 104-week warranty, 8% of the cover is spent before a patient is treated. That gap moves the risk from the contractor to the hospital. According to 21 CFR 820.181(e), a device master record must include or point to installation, maintenance and servicing procedures. Those procedures are the spine of the operations and maintenance manual. In our own handover packs, we found the index page is the first page a biomedical team opens. The 2026 annex makes the index a release condition. No index, no final payment.

Which Clause Lines Can Be Copied Into the Order?

A clause line is one sentence that states a number, an obligation and a consequence. Five lines carry the whole annex into the contract. Each line names a carrier, the channel and the result of a missed date. According to ISO 13485:2016 clause 4.2.4, documents of external origin are identified and their distribution is controlled. A clause line is how a hospital applies the same rule to a supplier. Three of the five lines carry a payment consequence, so 60% of the annex has money behind it. The other two carry a schedule consequence. A line without a consequence is a wish. In practice, buyers who paste five lines into the order spend less time arguing in week twenty than buyers who paste none. A buyer can test every line against one row of the register.

Clause Wording to Paste Into the Order

The five clause lines below are written to be pasted into the order without editing.

Line Wording Consequence
1 Only documents carrying a PRJ-DS, PRJ-FAT, PRJ-RIF, PRJ-UR or PRJ-CL number are contractual Unnumbered copies carry no contractual effect
2 Revision status closes as A, B or C within ten working days of comment A missed window holds the release
3 Each unit file is keyed to the serial number on the machine plate A file without a serial is rejected at handover
4 Factory acceptance signature binds one serial, not one model Unwitnessed units ship without acceptance
5 Closeout index is a condition of final payment Final payment holds until the index is approved

How Should the Annex Be Tested Before Award?

The test is a dry run of the register with one sample unit and one sample room. A dry run uses real papers, not a presentation. Step 1 numbers a real datasheet from the bidder. Step 2 dates one revision round. Step 3 fills one factory acceptance record from a past test. Step 4 signs one unit record in the fixed order. According to 21 CFR 820.181, a device master record must include or point to the location of acceptance criteria. The dry run checks whether the bidder can point to a location. Ten sample documents are 25% of a forty-document pack, which is enough for the test. The four steps are the whole test, and each one leaves a paper behind. The 2026 exercise takes about four hours. A bidder who cannot complete four steps in four hours will not complete four hundred units in four months.
