Electromagnetic compatibility — EMC — is one of the most misunderstood requirements for medical device importers. When a piece of electrical medical equipment enters the European Economic Area, it must comply with IEC 60601-1-2, the EMC standard for medical electrical equipment. This guide explains what the standard covers, what tests are involved, and what importers should look for in test reports before placing an order.
What Is IEC 60601-1-2?
IEC 60601-1-2 is the collateral standard for EMC within the IEC 60601 series. While IEC 60601-1 covers general safety requirements for medical electrical equipment, the -1-2 standard addresses the electromagnetic environment. It sets two types of requirements: emissions (how much electromagnetic noise the device produces) and immunity (how well the device continues to function when exposed to external electromagnetic interference).
The standard applies to all medical electrical equipment and systems, including surgical lights, operating tables with electronic controls, medical pendants with integrated monitors, hospital beds with electric actuators and infusion pumps. If the device has any electrical or electronic circuitry, IEC 60601-1-2 applies.
Emissions Tests
Emissions testing measures the electromagnetic energy that the device radiates or conducts back into the power supply. The two primary tests are radiated emissions (measured in an anechoic chamber or on an open-area test site) and conducted emissions (measured at the power cord using a line impedance stabilization network).
Radiated emissions limits depend on the intended use environment. Equipment for use in home healthcare environments faces stricter limits than equipment intended only for professional healthcare facilities. Importers should confirm with the manufacturer which use environment was assumed during testing.
Immunity Tests
Immunity testing exposes the device to controlled electromagnetic disturbances and verifies that it continues to operate within specification. Key immunity tests include electrostatic discharge (ESD), radiated radio-frequency immunity, electrical fast transients (EFT), surge, conducted RF immunity, power-frequency magnetic fields and voltage dips and interruptions.
For surgical lights and operating tables, the most relevant immunity test is often radiated RF immunity at 80 MHz to 2.7 GHz, at a test level of 3 V/m or 10 V/m depending on the intended environment. Equipment used near radio communication transmitters — for example, in hospitals with extensive Wi-Fi infrastructure — may need to pass the higher 10 V/m level.
What Importers Should Check in Test Reports
When a manufacturer provides an IEC 60601-1-2 test report, importers should verify several items. First, the test laboratory should be ISO 17025 accredited for EMC testing. Second, the report should cover the exact model and configuration being purchased — a report for a different model is not valid. Third, check the pass/fail criteria: the device must maintain basic safety and essential function during and after each test.
Fourth, verify the test levels. If the report shows only Group 1 (non-life-support) testing but the device is intended for life-support use, the report is insufficient. Fifth, check the date: IEC 60601-1-2 Edition 4.1 (2020) is the current version. Reports based on Edition 4.0 or earlier may not be accepted by notified bodies.
EMC and the CE Marking Process
For CE marking under the EU MDR, IEC 60601-1-2 compliance is mandatory for all electrical medical devices. The notified body will review the EMC test report as part of the technical file assessment. If the report is missing, incomplete or based on an outdated edition, the conformity assessment will be delayed.
Importers should request the IEC 60601-1-2 test report early in the sourcing process, ideally before placing a sample order. Practical Tips for Importers
Always request the full test report, not just a summary certificate. Verify the laboratory accreditation on the ILAC MRA database. Confirm that the tested configuration matches the product you are ordering. And keep a copy of the test report in your technical file — you may need it for regulatory audits or tender submissions.
